    Health History Form
    Home Language Survey
    Internet Access Release Form

    These forms are optional:

Only fill out these forms if applicable.

    Transfer Student Athletics Form

  • Return the completed packet to the counseling secretary.  At this time, please provide:

    1.  a copy of the student's current transcript and/or withdrawl grades
    2.  immunization records
    3.  a copy of current WASL scores or other state standardized test scores
    4.  proof of residence within the proper school boundaries

A current utility bill or rental/mortgage agreement is acceptable.  A driver's license cannot be accepted.  If you are unsure as to within which district or school boundary you reside, contact transportation

  • Read over attendance procedures and student code of conduct

  • Wait for a call from the counseling secretary.  Before students are admitted, records will be requested from the former school(s), and an administrator will complete an attendance and discipline check.  Once the administrator has approved enrollment, you will be called and a registration appointment will be set.  This process may take several days.  Please be patient as we collect all of the necessary information.  This check helps us ensure the safety of all of our students and staff.

  • Meet with the counselor who will be assigned to your student to discuss graduation requirements and an educational plan for the student.  Students are assigned to specific counselors according to the spelling of the student's last name.

Some answers to frequently asked questions:

How do I get an ASB card and annual?

ASB cards and annuals may be purchased from the bookkeeper.  When you come to register, please make the bookkeeping office one of your stops.

How do I find transportation information?

The transportation office can help you with finding the bus number, stop nearest your home, and where in the line-up at school the student will find the bus.  In addition, the transportation department will update the bus schedules online.  Please click here.

What if my child wants to drive?

Parking is available to seniors first and on a first-come-first-served basis.  See the bookkeeper for the parking form and to pay for a permit.  Once the seniors have received their permits, a lottery is held, and some juniors receive permits.  If the student is a junior, please see the bookkeeper to be placed on the lottery list. Sophomores and Freshmen will not be issued permits.

Do you offer driver's education?

My child is an athlete.  How do help get him or her involved in sports activities?

Please fill out the transfer student athletics form in the enrollment packet and contact the athletic office.  They will help you with any other necessary paperwork, eligibility information, and contact information.  Please also inform the counselor.  Student athletes who plan on attending Division I and Division II schools have curricular requirements that must be met.
